Immunizations

Quincy Public Schools believes that a healthy and safe learning environment is essential for student success. By following state and federal guidelines, we ensure that our students are protected against preventable diseases and foster an atmosphere conducive to academic excellence.

The Massachusetts General Laws Chapter 76, Section 15 and Chapter 71, Section 57 requires all students to submit a recent physical examination (within last 12 months) and provide proof of immunization against DTP, Hepatitis B, Polio, Measles, Mumps, Rubella, Varicella, and TB assessment/test as a requirement for school entry.

In order for your child to begin school we will need documentation of the following:

Pre-School

  • Lead Screening (date and results)

  • 1-4 doses of HIB

  • 4 doses of DTP Vaccine

  • 3 doses of Hepatitis B Vaccine (5mcg)

  • 3 doses of Polio Vaccine

  • 1 dose of MMR Vaccine (Measles, Mumps, and Rubella)

  • 1 dose of Varicella Vaccine (physician diagnosis of chicken pox, or serologic proof of immunity)

  • TB Assessment

K – 6th Grade

  • Lead Screening (date and results/K only)

  • 5 doses of DTP Vaccine (4 doses if last dose given on/or after 4th birthday)

  • 4 doses of Polio Vaccine (3 doses if last dose given on/or after 4th birthday)

  • 3 doses of Hepatitis B Vaccine (5mcg)

  • 2 doses of MMR Vaccine (Measles, Mumps, and Rubella)

  • 2 doses of Varicella Vaccine (physician diagnosis of chicken pox, or serologic proof of immunity)

  • TB Assessment

7 -12th Grade

  • 5 doses of DTP Vaccine

  • 1 dose of Tdap (if it has been > 5 years since last dose of DTap/DTP/DT)

  • 3 doses of Hepatitis B Vaccine (5mcg) or 2 doses of Hepatitis B Vaccine (10mcg)

  • 4 doses of Polio Vaccine (3 doses if last dose given on/or after 4th birthday)

  • 2 doses of MMR Vaccine (Measles, Mumps, and Rubella)

  • 2 doses of Varicella Vaccine (physician diagnosis of chicken pox, or serologic proof of immunity)

  • 1 dose MenACWY (Menningococcal) Vaccine (Grades 7-10)

  • 2 doses MenACWY (Menningococcal) Vaccine (Grades 11-12)

  • TB Assessment

The nurse will review the medical documentation for compliance before a student is allowed entry into school. Thank you.
